Repair intake form
An intake sheet with the device, the fault, accessories and condition — printed for the customer.
The intake form is the main document of the handover. It protects both sides: it records what was brought in, in what condition and what was agreed.
What is recorded at intake
- device: type, brand, model, serial number, product code;
- the fault as described by the customer and the type of repair — warranty, paid, repeat;
- accessories and condition: scratches, cracks, missing screws;
- customer: name, phone, email, address, company;
- urgency and intake date.
Printed on your letterhead
The form prints from your own template: shop name, company details, intake terms. The customer leaves with a sheet carrying the job number they can check later.
Faster intake
For regular customers or identical devices you can copy the previous job: reference data is filled in, and you only change what differs.
